Transition House - Brockton STARR
The Old Colony Y Transition House provides services for youths, male and female, ages 12-18; who are referred by the Department of Children and Families. The Old Colony Y collaborated with the Justice Resource Institute to provide a wide range of services to our youth, utilizing resources from within the community to maintain the connection between our youth and their community.
This program works with youth who are looking to successfully transition back to their home following treatment, or to an alternate environment that is more appropriate for their needs. Family involvement is a major component of the program and is encouraged through on-site visits, day passes, and overnight passes, to be determined by each individual social worker. Youth remain with the program for approximately forty-five days. During that time, various services may be utilized, such as: individual therapy, family therapy, anger management, life skills, substance abuse awareness, and other educational groups. Youth in the program are also provided the opportunity to participate in recreational activities facilitated by program staff during evening hours and on weekends.
The Transition House is committed to providing a safe and nurturing environment, which presents the residents with the opportunity to successfully transition into an appropriate placement in an additional residential program, foster care placement, or back home and in their community.
